The Sports Coordinators' contact information is on the individual sport pages on the website.

3. Complete and submit the following three forms:
A Worker Registration Acknowledgement Form
B Worker Registration form
C Commitment to Ethical Conduct form

All assistant coaches must submit to the requirements above, with the exception of having to attend the Coaching to Make a Positive Difference seminar and obtaining a Head Coach ID card. In lieu of a Head Coach ID card, assistant coaches must obtain a CYC Photo ID card. Assistant coaches who have a Head Coach ID card need not also have a CYC Photo ID card.
